Live Well, Be Well was a community-based lifestyle program to reduce risk for diabetes and designed specifically for English and Spanish-speaking lower socioeconomic status and minority adults at risk of developing diabetes.
Live Well, Be Well consisted of a 6-month active intervention phase and a 6-month maintenance phase. Trained public health department counselors provided participant-centered support, education, and skills training to modify diet and physical activity primarily through telephone-based counseling (12 calls), with 1 in-person session, and 6 group workshops. In-person session and group workshops were held in neighborhood settings.

The project was a partnership between the University of California San Francisco (UCSF) and the City of Berkeley Division of Public Health. It was funded by a translational research grant (R18 DK067896) from the National Institute of Diabetes and Digestive and Kidney Diseases.
