We describe the outreach and diabetes screening procedures which include step-by-step guides and materials. Materials are available in English and Spanish. Outreach and diabetes screening can provide people in underserved communities with diabetes health education; and is an optional component that can be done with or without the Live Well, Be Well program. Although these were integrated, the information is presented in two sections:
