Program Manual

The Live Well, Be Well program manual and supporting documents are provided for each program component.  Program components are:

  • Get Ready, Get Set: Prevent Diabetes - an Introductory Workshop

  • Personal Planning Session
  • Telephone Calls
  • Group Workshops

Program materials were developed to appeal to people with low literacy and created to be culturally and linguistically appropriate.  Program materials are available in English and Spanish.

In using the program manual:

  1. Relevant materials and forms are noted in yellow boxes, and are available in the appendix. 
  2. Blue boxes refer to relevant pages in the “Participant Program Binder”.
  3. Materials are formatted in Microsoft Word, unless indicated.

Program Manual

Get Ready, Get Set: Prevent Diabetes Introductory Workshop introduces the Live Well, Be Well program and provides a diabetes educational workshop; it is facilitated by a counselor or program staff.

The Participant Program Binder is used by participants during group workshops or to refer to during phone calls.  It was designed to be a stand-alone document that provided the essentials to the Live Well, Be Well program.  It is divided into six sections and includes behavior-specific content and suggestions.

The Personal Planning Session (PPS) is the only one-on-one meeting.  At this session, participants and their counselor discuss participants’ long-term goals and help them create short-term goals or action plans.  Because the PPS is based on each participant’s readiness for change and goals, this protocol is written with key discussion points outlined instead of a scripted step-by-step protocol.  This PPS protocol was used for both English and Spanish-speaking participants.

Telephone Calls  were made by counselors, and provide ongoing participant-centered support and skills training.  Telephone calls are scheduled atparticipants’ convenience; 10 calls are made in the first 6 months (active program phase) and 2 calls are made in the second 6 months (maintenance phase).  Because telephone calls are based on participant’s readiness for change and goals, this protocol is written with key discussion points outlined instead of a  scripted step-by-step protocol.  This telephone calls protocol was used for both English and Spanish-speaking participants.

Four Group Workshops were offered.  Participants were encouraged to attend at least two workshops during the first 6 months (active program phase) and to attend any missed workshop during the second 6 months (maintenance phase).  English and Spanish workshops were offered monthly at convenient times, and facilitated by a counselor or program staff.

Graduation and Strategies for Healthy Living Workshop is for participants who have completed the 6-month active program phase.  It is offered on a needs only basis (when a small group completed the program).

Training and Staffing for the Live Well, Be Well program was delivered by the public health department staff.  Counselors provided ongoing participant-centered support and skills training to participants.  Other health department staff delivered the workshops, for example, health educators and community health workers.  A program coordinator provided overall administration including community liaison, training and ongoing staff support.  We describe basic roles for program staff and skills needed to fulfill these roles.  Organizations thinking of adapting Live Well, Be Well may need to modify staffing to take advantage of their own resources and meet the needs of their community.
